Equipment Rental Notice
 
Medicare will only pay for the rental of this piece equipment for 13 months.  By law, Medicare will not “purchase” this equipment but ownership will transfer to you in month 14.
 
Medicare will make rental payments on this equipment for 13 months.  If or when the equipment is no longer needed, or if the patient enters a nursing home, the equipment will need to be returned to Restoration Medical Supplies, Inc. immediately.  Call to arrange pick up.
 
Please note that by signing Restoration Medical’s paperwork you accept all responsibility for loss or damage while this equipment is in my custody.  You also understand that when rental equipment is not returned, as agreed, this is defined as “Theft of Service” under New York state law. 
 
Medicare beneficiaries – please note that as of November 1st, 2008, Medicare will only pay for your CPAP for between 90 – 120 days.  During this time you must show a therapeutic benefit from the device as measured by a) usage of four hours per night or more for more than 75% of the days in a 30 day period, b) a face to face meeting with you sleep doctor prior to your 120th day where he/she confirms your usage and that the therapy is supplying a therapeutic benefit to you.  Otherwise it is your responsibility to return the device to us
 
 
PRIOR RENTAL/PURCHASE HISTORY
As part of the Medicare requirements, Restoration Medical will ask you to certify that you have never rented or purchased the same or similar equipment provided to you today under the Medicare Program
